This report is intended for moderate injuries or illness (parent(s) picked up child, transported to doctor or hospital) to serious injuries or severe illness (911 called and injured person transported to a hospital) involving a student or visitor.
This report is for internal use only because it is attorney/client privileged and it should not be copied or distributed. Follow your district policies and procedures.
Basic Incident Report (Parent Report)
In the event of a student or visitor injury or serious illness, you may use this report to document the incident and a copy of this report may be given to the parent or visitor by request only.
